Medication Review
You are eligible for the Medication Review program if you have a valid Ontario Health Insurance Plan (OHIP) card and if any one of these is true:
- You take three or more prescription medications for a chronic condition.
- You have been diagnosed with Type 1 or Type 2 diabetes.
- You live in a licensed long-term care home.
- If the need arises, you may also be eligible for one or more follow-up appointments throughout the year.
What do I bring to the service?
Any medications that you are taking should be brought to your scheduled Medication Review. This includes prescriptions and over-the-counter medications, vitamins and natural health products. Our pharmacist will consider each of your medications and discuss with you how to achieve the best health outcomes.
If you are eligible for the Medscheck program but are not able to leave your home or LTC residence to visit our pharmacy, please speak to your pharmacist about the Medication Review at Home or LTC program.
